Pharmachologic effect:

Topiramate is an anticonvulsant medication that works by stabilizing electrical activity in the brain, reducing the frequency and severity of seizures. It also has mood-stabilizing properties and is used in the prevention of migraines.

Indications:

• Epilepsy: for the treatment of partial onset seizures, primary generalized tonic-clonic seizures, and seizures associated with Lennox-Gastaut syndrome.
• Migraine: for the prophylaxis of migraine headaches.

Contraindications:

• Hypersensitivity to topiramate or any of the excipients in the formulation.
• Pregnancy, due to potential harm to the fetus.
• Metabolic acidosis with concomitant use of metformin.

Side effects:

In order of decreasing probability: Very common: dizziness, fatigue, paresthesia, weight loss., Common: depression, cognitive disorders, nausea, diarrhea, somnolence., Uncommon: kidney stones, metabolic acidosis, decreased sweating., Rare: glaucoma, severe skin reactions, liver failure.

Dosing and Administration:

For epilepsy, the initial dose is typically 25-50 mg per day, which can be increased weekly by 25-50 mg until the effective dose is reached, usually between 100-200 mg per day in divided doses. For migraine prophylaxis, the recommended dose is 100 mg per day, administered in two divided doses. Dosage adjustments may be necessary based on clinical response and tolerability.
