Quetiapine

Pharmachologic effect:

Quetiapine is an atypical antipsychotic that works by modulating neurotransmitter activity in the brain, primarily affecting serotonin and dopamine receptors. It is used to manage symptoms of schizophrenia and bipolar disorder.

Indications:

• Schizophrenia
• Bipolar disorder
• Major depressive disorder (as adjunct therapy)

Contraindications:

• Hypersensitivity to quetiapine or any of the excipients
• Concomitant use with cytochrome P450 3A4 inhibitors

Side effects:

In order of decreasing probability: Drowsiness, Dizziness, Dry mouth, Constipation, Weight gain, Increased appetite, Orthostatic hypotension, Elevated cholesterol levels, Elevated triglyceride levels, Extrapyramidal symptoms, Tachycardia, Blurred vision

Dosing and Administration:

The dosage of quetiapine should be individualized based on the patient's clinical response and tolerability. For schizophrenia, the usual starting dose is 25 mg twice daily, with increments of 25-50 mg two or three times a day on the second and third day, as tolerated, to a target dose of 300-400 mg daily by the fourth day. For bipolar disorder, the initial dose is 50 mg twice daily, increasing to 100 mg twice daily on the second day, 150 mg twice daily on the third day, and 200 mg twice daily on the fourth day. The maximum recommended dose is 800 mg per day.

Storage:

Store at a temperature not exceeding 30°C (86°F).
