Pharmachologic effect:

Verapamil is a calcium channel blocker that works by relaxing the muscles of the heart and blood vessels. It reduces the workload on the heart and helps it to beat more regularly.

Indications:


  • Hypertension
  • Angina pectoris
  • Supraventricular tachycardia
  • Atrial fibrillation or flutter

Contraindications:


  • Severe left ventricular dysfunction
  • Hypotension (systolic pressure less than 90 mm Hg)
  • Cardiogenic shock
  • Sick sinus syndrome (unless a functioning pacemaker is present)
  • Second- or third-degree AV block (unless a functioning pacemaker is present)
  • Wolff-Parkinson-White syndrome
  • Lown-Ganong-Levine syndrome
  • Known hypersensitivity to verapamil

Side effects:


In order of decreasing probability: Constipation, Dizziness, Nausea, Headache, Fatigue, Bradycardia, Hypotension, Peripheral edema, Heart failure, Elevated liver enzymes

Dosing and Administration:


The usual starting dose is 80 mg to 120 mg taken orally three times a day. The dose may be adjusted based on the patient's response and clinical condition. Maximum daily dose should not exceed 480 mg.

Storage:


Store at a temperature not exceeding 25°C (77°F).
