Losartan is an angiotensin II receptor antagonist that works by blocking the binding of angiotensin II to the AT1 receptor, which results in vasodilation, reduced secretion of vasopressin, and decreased production and secretion of aldosterone. This leads to a reduction in blood pressure and a decrease in the workload on the heart.Indications:
- Hypertension
- Heart failure
- Reduction of cardiovascular risk in patients with left ventricular hypertrophy
- Diabetic nephropathy in patients with type 2 diabetes and a history of hypertension
Contraindications:
- Hypersensitivity to losartan or any of the excipients
- Severe hepatic impairment
- Pregnancy
- Concomitant use with aliskiren in patients with diabetes
Side effects:
In order of decreasing probability: Common: dizziness, upper respiratory infection, nasal congestion, Uncommon: hypotension, fatigue, hypoglycemia, Rare: angioedema, liver function abnormalities, hyperkalemia
Dosing and Administration:
The usual starting dose is 50 mg once daily. The dose may be increased to 100 mg once daily based on blood pressure response. In patients with possible intravascular volume depletion (e.g., those treated with diuretics), a starting dose of 25 mg once daily should be considered.
