Torasemide

Pharmachologic effect:

Torasemide is a loop diuretic that inhibits the Na+/K+/2Cl- carrier system in the thick ascending limb of the loop of Henle, leading to increased excretion of sodium, chloride, and water. This results in a reduction of fluid overload and edema associated with heart failure, renal disease, and liver disease.

Indications:

Treatment of edema associated with congestive heart failure
Edema due to renal disease
Edema associated with hepatic cirrhosis
Management of hypertension

Contraindications:

Hypersensitivity to torasemide or sulfonylureas
Anuria
Hepatic coma
Severe electrolyte imbalance

Side effects:

In order of decreasing probability: Common: Hypokalemia, headache, dizziness, Uncommon: Hypotension, dehydration, nausea, Rare: Tinnitus, rash, renal impairment, Very rare: Anaphylactic reactions, severe skin reactions

Dosing and Administration:

For edema, the usual starting dose is 5 mg once daily, which may be increased to 10 mg once daily if necessary. For hypertension, the recommended dose is 2.5 mg once daily, which may be increased to 5 mg once daily if required. The tablets should be taken orally with or without food.

Storage:

Store at a temperature not exceeding 25°C (77°F).
