Eplerenone is a selective aldosterone blocker that works by inhibiting the binding of aldosterone to the mineralocorticoid receptor, which reduces sodium reabsorption and water retention, leading to a decrease in blood pressure and a reduction in the workload on the heart.Indications:
- Heart failure post-myocardial infarction
- Hypertension
Contraindications:
- Hyperkalemia
- Severe renal impairment
- Severe hepatic impairment
- Concomitant use with strong CYP3A4 inhibitors
- Hypersensitivity to eplerenone or any of the excipients
Side effects:
In order of decreasing probability: Hyperkalemia, Dizziness, Fatigue, Diarrhea, Cough, Nausea, Headache, Gynecomastia
Dosing and Administration:
The recommended starting dose is 25 mg once daily, which may be increased to a target dose of 50 mg once daily, preferably within 4 weeks, based on the patient's response and tolerance. Dose adjustments may be necessary based on serum potassium levels and renal function.
