Eplerenone works by blocking the action of aldosterone, a hormone that regulates blood pressure and fluid balance in the body. This medication helps improve heart function and reduce the risk of hospitalization for heart failure. Eplerenone is suitable for patients who have experienced a heart attack or have systolic heart failure. It is typically prescribed alongside other heart medications to enhance treatment efficacy.

Pharmachologic effect:

Eplerenone is a selective aldosterone blocker that works by inhibiting the binding of aldosterone to the mineralocorticoid receptor, which reduces sodium reabsorption and water retention, leading to a decrease in blood pressure and a reduction in the workload on the heart.

Indications:


  • Heart failure post-myocardial infarction
  • Hypertension

Contraindications:


  • Hyperkalemia
  • Severe renal impairment
  • Severe hepatic impairment
  • Concomitant use with strong CYP3A4 inhibitors
  • Hypersensitivity to eplerenone or any of the excipients

Side effects:


In order of decreasing probability: Hyperkalemia, Dizziness, Fatigue, Diarrhea, Cough, Nausea, Headache, Gynecomastia

Dosing and Administration:


The recommended starting dose is 25 mg once daily, which may be increased to a target dose of 50 mg once daily, preferably within 4 weeks, based on the patient's response and tolerance. Dose adjustments may be necessary based on serum potassium levels and renal function.

Storage:


Store at a temperature not exceeding 30°C (86°F).
