Pharmachologic effect:

Sotalol is a non-selective beta-adrenergic receptor blocker that exhibits both class II and class III antiarrhythmic properties. It works by prolonging the cardiac action potential duration and refractory period, thereby stabilizing cardiac rhythm.

Ventricular arrhythmias
Atrial fibrillation

Contraindications:

Bradycardia
Second or third-degree AV block
Congenital or acquired long QT syndromes

Asthma or severe chronic obstructive pulmonary disease
Hypersensitivity to sotalol

In order of decreasing probability: Fatigue, Dizziness, Bradycardia, Dyspnea, Nausea, Hypotension, Proarrhythmia, Heart failure, Cold extremities, Depression

Dosing and Administration:

The recommended initial dose is 80 mg twice daily. The dose may be adjusted based on patient response and tolerance, with a maximum dose of 320 mg per day. It should be taken on an empty stomach, at least 1 hour before or 2 hours after meals.
