Simvastatin is a lipid-lowering agent that works by inhibiting HMG-CoA reductase, an enzyme involved in the synthesis of cholesterol in the liver. This leads to a decrease in total cholesterol, low-density lipoprotein (LDL) cholesterol, and triglycerides, while increasing high-density lipoprotein (HDL) cholesterol.Indications:
- Hypercholesterolemia
- Mixed dyslipidemia
- Primary hyperlipidemia
- Prevention of cardiovascular events in patients with high risk
Contraindications:
- Hypersensitivity to simvastatin or any component of the formulation
- Active liver disease or unexplained persistent elevations of serum transaminases
- Pregnancy and lactation
- Concomitant use with strong CYP3A4 inhibitors
Side effects:
In order of decreasing probability: Headache, Abdominal pain, Constipation, Nausea, Muscle pain, Increased liver enzymes, Rhabdomyolysis (rare)
Dosing and Administration:
The recommended starting dose is 10-20 mg once daily in the evening. For patients requiring a large reduction in LDL cholesterol, the starting dose is 20-40 mg once daily in the evening. The dosage should be individualized according to the patient's response and should not exceed 40 mg per day.
